Patient Review Instrument (PRI) Services

Umbrella Care Management offers PRI & SCREEN Assessments across the country. This assessment is required to be conducted on patients prior to their admission to any nursing home in New York State.

PRI & SCREEN assessments must be completed by a PRI-certified professional, usually a nurse or social worker.

For families seeking to relocate a loved one to a New York facility from another part of the country, completing a PRI & SCREEN assessment can be extremely difficult due to the lack of qualified professionals outside New York.

Umbrella Care Management has PRI-certified nurses available to conduct these assessments across the country.
